Botox Effects on Cerebral Palsy

Botox might best be known for its use as a cosmetic procedure, but studies are showing that Botox, when used in small doses on cerebral palsy patients, can prove useful in the treatment of muscle spasms suffered by cerebral palsy patients.

Spasticity is commonly found in people with cerebral palsy. It is caused by increased muscle tone and those with spasticity typically experience muscle weakness and unusual reflexes.

Though Botox has been used for some time to treat cerebral palsy, the FDA cautioned against the practice until further research and evidence comes forward to the benefits of using Botox on cerebral palsy patients and the risks are mitigated.  Botox is typically used to remove wrinkles and is not approved by the FDA for treatment of cerebral palsy. Physicians, however, can use any medicine “off label” as long as the patient is consented first.  Indeed, Botox was used in this manner for years to treat facial wrinkles before receiving approval as “effective” by the FDA.

While this does shed some exciting possibilities for the treatment of cerebral palsy, continued research is needed to fully understand  how Botox can effectively treat cerebral palsy patients without causing any further complications or toxicity.

LaViv: Beauty From Within

What is LaViv?

LaViv is the latest addition to our injectable treatments. LaViv is relatively new on the market and is quite different from other injectables. With LaViv we take a biopsy of your skin tissues and we send it off to be processed at the LaViv laboratory. At the LaViv laboratory they take the tissue and process the fibroblasts, or cells that produce collagen, permitting them to be grown and concentrated. After 3 months, they are sent back to our clinic and are injected into our patients within 24 hours. Concentrating the fibroblasts takes about 3 months, producing 3 syringes worth.  The material can be stored at the LaViv lab until the patient is ready to be re injected with their own cells. 


How Long Does LaViv Last?

Although LaViv is not a permanent solution, the results last far longer than traditional injectables such as Juvederm, Radiesse and Restylane. We are not sure how long exactly LaViv lasts but because it is your own collagen, we feel it will continue to grow and produce results for at least  3-5 years. 

Is There Any Downtime?

Like most other injectables, LaViv is applied in a painless method with a minimal chance of  swelling and bruising. After the treatment, there should be no downtime. You should be able to return to your normal activities right after your treatment.

What is Botox - is Botox Safe?

 

What is Botox?

Botox is a neurotoxin from bacteria that is injected in very small, safe doses into the muscles of facial expression. The muscles in your face continuously contract producing your own unique facial expression. Over time these areas develop fine lines and wrinkles due to that continuous contracting.  Botox prevents the muscles from contracting, allowing the treated areas to become smoother.  Although Botox is a toxin, when used in small amounts, it only affects and debilitates the muscles it is injected into. Botox has been safely used for 15 + years with over 11 million injections performed.

What are the most common areas treated by Botox?

Botox is most commonly used to treat the facial expression areas of the face. Approximately 80% of our Botox treatments are performed on the frown lines between your eyebrows, the horizontal lines on your forehead, and the smile lines (crows feet) on the outside corners of your eyes. Other applications include lifting the brows, improving the appearance of the vertical creases of the upper lip and relaxing the muscles that turn down the corners of the mouth. Neck bands can also be softened.

Are Botox Injections Painful? What is the Downtime?

Botox Injections are virtually painless. We use a very small 31 gage needle that is inserted in and out quickly. Rarely patients request further numbing with an anesthesia cream or ice. Botox treatments are done quickly in our office and our patients are able to return to normal activities right after their treatment without any downtime.

What is a Liquid Face Lift?

As we age, we find that our skin becomes less firm and our tissues begin to sag. For many years it was believed that this was the result of skin becoming loose by losing elasticity, but we now understand that the loss of volume in the face is also a major factor in the skin's appearance. Decrease in fatty tissue, collagen as well as skin thickness all play a role in the aging process. Similar to a balloon deflating and the surface area wrinkling, the face loses volume and facial skin is affected in a similar manner. 

 

The Liquid Face Lift addresses this problem by replacing the lost volume. Unlike a traditional facelift that surgically pulls the tissues tighter, the Liquid Face Lift is a non-surgical treatment where a liquid media is injected into the face. These procedures rejuvenate the skin with a 3D effect that lifts and fills out the tissues giving the facial contours a smoother, rounder and younger appearance. 

 

There are a variety of injectable fillers that are utilized in a Liquid Face Lift. Juvaderm and Restylane are composed of microscopic particles of hyaluronic acid which is produced naturally in the skin. They are used to fill the treated area and restore the volume. Because these fillers are part of the collagen naturally found in our skin, they are also naturally broken down; meaning results are not permanent but last anywhere from 6 to 9 months. Other fillers such as Radiesse, made of microscopic particles of calcium hydroxyappetite (found in bone), are injected as volume filler, but also stimulate collagen growth. Sculptra, a form of poly-latic acid (the same material used in absorbable sutures), also stimulates collagen growth, restoring volume with a natural appearance and feel. Although Sculptra requires multiple treatments over time, the results last greater than 3 years.   Another option involves harvesting fat cells from your own body through liposuction, then injecting those cells to restore tissue along with volume. Since fat also contains stem cells, this process is sometimes referred to as a “Stem Cell Lift”

 

Because a Liquid Face Lift is non-surgical, the down time in comparison to a traditional face lift is dramatically minimized. A patient receiving a Liquid Face Lift will be able to leave immediately post procedure, returning to normal activities without delay, and may only experience very minor bruising and swelling if any.

Lots of folks didn’t know it – but when they read Dr. Sherman’s latest blog, he said that liposuction can be healthy for patients. That brings us to this update:

 

What Kind of People Are Doing Liposuction?

 

Dr Sherman says text books used to talk about the ideal liposuction patient. A patient that was in good shape – not overweight – but had some mild areas of resistant fatty fullness resistant to diet and exercise. “And so, liposuction was excellent for those patients, because their skin tone was good and it wasn’t a major procedure – kind of like touch-up work.”

While that sounds wonderful – those patients have taken a back seat. For most top lipo doctors – “We now see patients who have overeaten and gradually weight has been added in the form of fat deposits.” Most of those deposits align themselves in superficial areas such as the abdomen, hips, flanks, thighs, arms – even breasts. “As long as the fatty deposits are superficial – they’re approachable with liposuction directly,” says Dr. Sherman.

 

LIposuction is Permanent

 

One of the main advantages of lipo – is that it is permanent.  ”Not only are we removing fat – but removing fat cells. Those don’t reproduce after a certain age – around early adulthood – affecting a permanent change.

 

Other types of liposuction patients are those who have lost a tremendous amount of weight through diet or exercise or have gone through some sort of bariatric  surgery. Gastric stapling, gastric bypass or even a lap-band procedure, fall into this category. After a significant amount of weight loss, there’s stabilization. These patients tend to get liposuction to continue their improved body image – or perhaps combine further fat removal with a body contouring procedure such as a tummy tuck, arm lift or breast lift


Liposuction Can Be Used In Brest Reduction

 

The other interesting segment of the lipo population – is older female patients who have unfortunately, developed a lot of weight in the breast. “We can do a wonderful job in reduction of the breast through liposuction only – and relieve the back pain or shoulder pain without reconstructive surgery of the breast with its inherent scarring or distortion of the breast shape.”

The last area of liposuction patient we see, particularly this time of year is bride grooms, members of the bridal party and their families – in order to look their best. You want to do this about a month before the big ceremony.

So overall – the amount of different patient categories that fit-in with liposuction have signicantly expanded. No longer are we looking at the patient who wants minimal liposuction.

Recently, San Diego’s Dr Maurice Sherman attended an international meeting of doctors concerned with techniques and advances in body contouring that followed massive weight loss. “This is becoming more common and there’s much more awareness due to TV shows such as, ‘The Biggest Loser.’” Also, says Dr Sherman, because so-called bariatric medicine/surgery – (medicine and surgery for weight loss,) is becoming much more safe and mainstream. Insurance is now covering medical therapy because it increases quality of life with lifestyle, longevity and decreased medical problems such as diabetes, arthritis, high blood pressure. “So, patients are given a new lease on life if they meet the indications  for undergoing some of these procedures such as lap bands and/or gastric stapling. The surgeries are being done with fast recovery times – and the safety of these procedures have vastly improved over 10 years ago.”                                                                                                                                                        

Continues Dr Sherman: “Then we have the issue – when they lose 50 to 100 pounds and stabilize, they have a problem of skin laxity – stretching of the skin. Depending on the person, you’ll see problems in the inner/outer thighs, abdomen, loss of lift of the breast – and back of the upper chest. And arms (batwings) and with, with older patients – there’s also laxity issues. Surgical treatments are much more safer, they’re outpatient procedures and have minimal scarring. Overall – we’re seeing a tremendous increase in demand for these types of procedures.”


Dr Sherman says he’s one of the few in the San Diego area with experience and training in this type of procedure. “A good example is a combination of tummy tuck and liposuction – called the lipo-abdominoplasty  or Avelar abdominoplasty. Here, the patient heals very fast – and are up and on their feet basically the next day. The recovery is as good as having liposuction.”

Dr Sherman says he’s very excited because of the capability of performing liposuction which is more thorough and produces a greater degree of skin contracture – and leaves the patient with a smoother body contour. Skin contracture – means the skin has to contract after you remove fat. It has to re-contour itself over the area of depleted volume. That can vary over skin/genetics and things like hormonal influence. There has always been an attempted to maximum the amount of skin contracture. Surgical incisions – can lead to scarring. And – in the past, lasers such as Smart Lipo were used to treat the skin – but that could lead to complications because of the laser heat. SAFE Vaser Liposuction only includes small, minimal surgical incisions which are cosmetically undetectable.  

The newer technique is a combination of the Vaser and fat disruption techniques. The Vaser stimulates skin contraction and loosens fibrous fat. The result is excellent – smoother skin and body contouring. It’s the best liposuction technique out there.

This liposuction technique produces a nicer/smoother contour with maximum amount of skin contraction without surgery and Dr Sherman can now able to treat difficult areas from older liposuction techniques including scarring – or   those with very fibrous, stiff and fatty areas. These are areas such as love handles, male breast enlargement and more –  are now very approachable. Yes, skin creases disappear with this new lipo technique.

San Diego Update…What is Ultherapy?

Ultherapy refers to treatment utilizing the ulthera appliance. The ulthera appliance or equipment is a new device that is effective in skin tightening. It is FDA approved for facial skin tightening, but there have been recent discoveries in utilizing this therapy in other areas of the body. These discoveries entail skin tightening for the elbows, knees and cleavage/decollete area also known as the upper part of the chest close to the breasts. The decollete area is particularly affected by sun damage and aging. Improving the decollete has always been a challenge due to the sensitive nature of the skin in this area. With Ultherapy however, we have a noninvasive and effective way of eliminating wrinkles and aging.


Non-Surgical Breast Lifting

Along with these new uses for Ultherapy, we have a new method for non-surgical breast lifting. This is done by using Ultherapy on the upper part of the breast. Preliminary studies show results of anywhere from one to two centimeters of breast lift. With repeated procedures we hope to produce even better breast lifting results.
